> For News Students – Report Card, Certificate of Good Moral, 3pcs. 2×2 picture, Certificate of Residency/Indigency, PSA Birth Certificate (Photocopy), Form 137.
> For Transferees – Certificate of Eligibility to Transfer, 3pcs. 2×2 picture, PSA Birth Certificate (Photocopy).
